Thanks for considering joining Lemons Inc. for your food service needs. Below are the forms that you need to join our program.

CACFP REQUIREMENTS:

  • Completion of  Food Services forms below

  • Current DHS License

  • ​Typed Master list of all enrolled children

  • ​Completed copies of income and enrollment forms

  • Signed Contract

  • ​ELRC Monthly Provider Statement ( For Profit Centers Only)

  • ​Copy of 501 (c) (3)  For Non Profits Centers

  • Complete Mandatory trainings (Approximately  2 hours)     

          - Meal count rosters, food serving amounts, civil rights, and regulations   (State, Federal and local)

  • Choose between reimbursement or delivery model   

          - Reimbursement is when the center does all of the shopping for its self and Lemons reimburses the total 

             cost of operating a food program, within the allotted budget.

          - Deliver model is where Lemons will deliver the products for you to prepare. You can have fresh products               delivered to your site that you cook.

          -You can choose precooked products to prepare (coming soon).

​Approval -  Once all the paperwork has been submitted to Lemons Inc. it will be considered for approval.  This process in its entirety may take several weeks.

At-Risk After-School Care Program

ARACP

​

Sites with an extracurricular or after school or weekend program can apply for snack and dinner free for the entire site depending upon the site location.



REQUIREMENTS:



  • Completed After School Food Service Application

  • Provide After School Enrollment list.

  • 501C or documentation of your non-profit status.

  • Complete mandatory training.  This session is approximately 2 hours with instructions on filling out Rosters and Meal Counts, serving amounts of food, and food handling according to state and local regulations.

 

​Approval -  Once all paperwork has been submitted, it will be considered for final approval.  This process in its entirety may take several weeks.

 Child & Adult Care Food Program

CACFP

​

Child and Adult Care Food Program (CACFP) is administered at the federal level by the USDA with oversight at the state level by the PA Department of Education, Division of Food and Nutrition.  The CACFP provides reimbursement for meals served to enrolled participants in non-residential child or adult care facilities.

 

Children and adults are eligible to participate if they are:

​

  • Birth to 12 years of age; or

  • Migrant children birth to 15 years of age; or

  • Ages 4-18 years of age for the At-Risk After-School Program

  • Birth to 18 years of age in Emergency Shelters; or

  • Special needs children over 12 years of age in care facilities primarily serving children under 18 years of age; or

  • Functionally impaired adults; or

  • Adults over 60 years of age


All for-profit and non–profit centers are eligible to apply for this program providing that they meet the proper requirements. Each meal follows the proper nutrition guidelines according to the USDA.
 

Summer Food Service Program

SFSP 

 

This program is open to children between the ages 1-18.  The program runs throughout the summer months of June, July, August, and sometimes even in September. 

 

Children who are eligible for this program may be entitled to receive at least 2 meals consisting of Breakfast & Lunch, or Lunch & Snack.
